Athletic Training Checklist for New/Transfer Athletes

1. ATS-Injury Tracking System

  • Create an account on ATS then follow the step-by-step instructions below.
  • To create an account by phone, click HERE
  • When the log-in page appears asking for ID and password use “NEW” for both (ID and Password) the first time. Then select agree to the pop-up that will appear. Another page will appear to input information such as name, team, year, etc. and you set the ID and password to your preference. Select “First year” student as the grade level. 
  • Save the page and more tabs will appear on the top, as you have successfully created a profile on the ATS system. (Once you have created a profile, use the “Returning Athlete Link” for subsequent log-ins. This link is to get the profile set up.)
  • Electronic Forms: Required to be completed are located under the “Forms” tab. Click on this tab. Next to Form name there is a drop down box. Only one form may be selected at a time. After selecting a form make sure to click on “NEW”. Complete this action for each form required.
  • Required for first-year/freshmen
    • 1. Medical History - New Athlete
    • 2. Release of Medical Information
    • 3. Wellfleet Medical Authorization
    • 4. Sickle Cell Waiver
    • 5. Assumption of Risk/Medical Authorization

**For the signature, use mouse/finger to sign name. Click "sign." Then click "save."

  • Insurance Tab: Click on the ADD button at the top. Fill out all information. Copy front and back of student’s primary health insurance card and upload to ATS. At the bottom of the insurance form there is a section to upload the cards. It works best to take a picture with a smartphone, e-mail it to yourself, save it to the computer and upload. If you have difficulty please contact one of the staff members to assist you. 

   
**Make sure insurance cards are horizontal and not vertical.

  • Sickle cell trait test results are required by the NCAA. You have 3 options: 1. Type in Test results (if have them, or contact the hospital). 2. You may waive your rights to be tested (most athletes usually opt for this). 3 You may get tested and report the results.

2. Physical/Medical History Forms

  • Simpson College should have mailed you forms to complete. These include: student heath form, Physician report, Immunization history. These forms can also be downloaded from the Simpson College website.
  • Please send a copy to Student Health Services as directed on the forms and one copy to Athletic Training- attention Taylor Witzel (Head Athletic Trainer).
  • We highly recommend that physicals are updated within six months of collegiate athletic seasons. We understand that due to insurance coverages this may not be possible, but this decreases the chances of missed practices or athletic events due to inability to obtain a new physical at the insurance time. Furthermore, the NCAA requires physicals to be signed off by a physician with a medical standing of M.D. (medical doctor) or D.O. (doctor of osteopathic medicine). Any physicals without a signature from an M.D. or D.O. risks the chance of not participating in NCAA athletics.

  • Student Health Services needs a copy of forms on file for every student. Only has to be submitted freshman/first year. Make sure you have vaccinations listed.
  • If the student continues to participate in athletics in subsequent years, a physical must be completed each year and sent only to Athletic Training.

3. Other Paperwork

  • If you have been diagnosed with ADHD or ADD, please see information located below. Documentation by physician is required.
  • NCAA Concussion form will be filled out once athlete has arrived on campus
